This Frequently Asked Questions page has been created as a part of Strategy I Communication of our district's Strategic Plan 2017-2022. Our goal with this page is to improve our website user experience by making it easier for visitors to find information quickly. Do I have to be on the district insurance to participate in the wellness incentive program?
No, the district encourages all employees to participate in the wellness incentive program.
How do I participate in the wellness incentive program?
Employees on the district health insurance plan who participate in the Biometric screening or submit a physician form and earn 200 points will receive $300 annually towards the following year medical premium contributions for engaging in the activities found HERE. Employees will be responsible for tracking and submitting their own wellness points. There will be tracking sheets as well as additional, required forms that will be made available at the Derby Public Schools webpage. All information that is submitted will be kept confidential.

How do I submit my wellness points earned?
Keep track of all wellness points earned (challenges, wellness checkups, etc.…) in a folder for your own records. Please see the staff wellness page on the Derby Schools website to access the backup forms. Keep all forms for proof of completion. At the end of September if you have not logged forms onto the Navigate web portal, all forms need to be sent to Jenny Ramsey in Human Resources. An audit will be done based off of those using the Navigate portal to make sure we have proof of completion. You do NOT have to use the Navigate portal (you may only use it if you are currently on the health insurance plan). The audit will be done in October. Once you have completed these steps, your name will be added to the list of those who may receive the $300 annual amount to put towards your medical premiums. What is a biometric screening and when can I participate?
Each year Derby Public Schools have Quest Diagnostics come to our buildings for biometric screenings. There are at least 4 different dates and locations for the screenings and you MUST be on the district health insurance to participate. A biometric screening measures some basic physical characteristics like your height and weight, blood sugar and cholesterol levels, as well as several other health indicators. These numbers provide a snapshot of your overall wellbeing.
Do I have to have the district health insurance to get a flu shot?
No, you can use whatever health insurance you have to participate in our on site flu shot clinic or you can use cash.
What time of year is the flu shot clinic available?
The district holds flu shot clinics on site at each building in October/November of each year to administer the flu shots. We bring in a local pharmacy to help, but you may also go to any local clinic or your physician for this service.
